Comprehensive Care Planning

Planning Your Care

A comprehensive care plan is developed by an experienced interdisciplinary team with the participation of the resident and/or representative. The plan includes measurable objectives and timetables designed to meet the individual residents medical, nursing, mental and psych-social needs.

 

Discharge Planning

A post-discharge plan of care is developed with the participation of the residents and his/her family. Discharge planning includes preparing the resident for the next level of care and arranging for placement in the appropriate care environment. The Interdisciplinary Team will complete an assessment and develop a planned program for discharge to a lower level of care, if medically feasible and realistic.

 

Medical & Nursing Care

Medical Care

Medical care is prescribed by the individuals attending physician. Our Medical Director acts as liaison between Alliance administration and all attending physicians and provides valuable consultation to the Director of Nursing in matters relating to nursing and rehabilitation services.

 

Nursing Care

Resident care is directed and supervised by a Director of Nursing with extensive experience in geriatric care. Registered Nurses work around the clock supported by a team of Licensed Vocational Nurses caring for our residents with pride and professionalism. State certified nursing assistants provide routine resident care and assistance with activities of daily living.

Who are we?
We are an independently owned, fully certified and licensed 139 bed skilled nursing facility at this location since the 1960’s.

Where are we?
Located in a quiet, residential neighborhood in El Monte.  Conveniently located near the 10 and 605 freeway intersection.  No appointment is necessary for a guided tour.

What do we offer?
Short-term to long-term care, 24/7 Registered Nurse on duty, Physical Therapy, Occupational Therapy and Speech-Language Pathology treatments as ordered by the physician in our rehab department, often, twice a day and six days a week.  IV therapies (TPN, antibiotic and hydration), pain management, wound care restorative and internal feeding programs, social services, varied activities, hospice and respite.

We feature culturally sensitive programs directed to the Asian population with interpreters, activities and menus.

Visiting hours?
10:00 AM to 8:00 PM.  Alternate visiting hours may be arranged.

What forms of payment do you accept?
Medicare, Medi-Cal, private pay and most Managed Care health plans.  Please call for more details!
